Understanding Your Benefits
Deductible
The amount you pay out-of-pocket before insurance begins covering services. Many plans reset on January 1.
Copay
A fixed amount you pay per visit (e.g., $30/visit) after your deductible is met.
Coinsurance
A percentage of costs you pay after your deductible (e.g., 20%). Your insurance covers the remaining 80%.
Out-of-Pocket Maximum
Once you hit this threshold, insurance covers 100% of covered services for the rest of the plan year.
Visit Limits
Some plans limit physical therapy to a set number of visits per year (e.g., 20 or 30). We can help you use them wisely.
Prior Authorization
Some plans require approval before starting PT. We handle the authorization process on your behalf.
Self-Pay & Out-of-Network
If you are uninsured, out-of-network, or prefer to pay directly, we offer transparent self-pay rates with no hidden fees. Many patients find that paying out-of-pocket for individualized, expert care is a better value than high-deductible insurance plans.
We accept all major credit cards, HSA/FSA cards, and cash. Superbills (itemized receipts for out-of-network reimbursement) are available upon request.
HSA & FSA
Physical therapy is an eligible medical expense for Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA). We accept HSA and FSA cards directly, making it easy to use your pre-tax healthcare dollars. If your FSA has a year-end use-it-or-lose-it deadline, contact us — we can often accommodate scheduling.
Billing Questions
How do I check my PT benefits?
Call the member services number on the back of your insurance card and ask: "What are my physical therapy benefits? Do I need a referral? What is my deductible and copay for outpatient PT?"
Will you verify my insurance before my first visit?
Yes. We verify your benefits before your initial evaluation so there are no billing surprises. We'll call you if we discover any issues.
What if my insurance denies a claim?
We handle appeals on your behalf. Our billing team has extensive experience navigating insurance disputes and getting claims approved.
